Tele-therapy appointments are easy to make at Dayspring. Sessions with a counselor at Dayspring requires up to 2 additional forms to be completed. If your health insurance has a copayment or you are using private pay the office will need a Credit Card Charge Approval form completed. The office staff will use this to take your payment. Dayspring also requires a signed consent to use Tele-therapy for treatment.
