We want to answer your questions and we have a few of our own if you are ready to schedule an appointment. Hopefully we can complete this in less than 5 minutes.


This is all we need to get you scheduled:


First and Last name

Date of birth

Phone number and email address

Type of health insurance and ID/Contract number

In-person or Teletherapy appointment

Day of the week and best time for appointment

Reason for the appointment such as Anxiety


The office staff will select the counselor who best meets your needs and help you select an appointment Date/Time to get you scheduled with the counselor.


From the information we gathered in your initial contact we will verify your insurance benefits and provider listed copayment in advance of your session. We want you to be informed of the costs you can expect to pay. While every insurance policy is different the majority of our clients pay less than $25 per session in copayments.


After we determine your estimated copayment we will send you a Welcome Email that contains a link to our HIPAA secure Electronic Health Record (EHR) known as CarePaths. Here you will complete your New Client Documents. These 5 documents help us finish preparing your charts and allows your counselor the opportunity to get to know you before you arrive.


New Client Documents can also be printed out and completed prior to your scheduled appointment. Please Fax the completed forms to our office at 256-878-8022. If you want to complete the forms when you arrive then please arrive approximately 25 minutes prior to your appointment.


Please arrive on time, we are expecting your appointment to begin when scheduled. We make a tremendous effort to eliminate needless waiting in the lobby because this just adds to the clients stress.


The office staff will help you to check in. On your first appointment the staff will need to scan your Driver License and Insurance Card. We ask that you make your copayment at this time. The staff will also offer you some refreshment that you are welcome to take into your session.


Your counselor will come to the lobby and greet you. You should expect the counselor to spend about 1 hour with you during a counseling session. Your counselor is an expert at ”active listening” and will ask you questions about why you’re there and what your goals are. You also have plenty of opportunity to ask your counselor questions.


At the end of the session you and your counselor will discuss a treatment plan based on what transpired during your session. You should expect to make a commitment to attend a minimum of 6 sessions to achieve lasting changes.


What about my privacy? Yes, the client/therapist relationship is one protected by law. Information disclosed during session is not released unless the client gives the therapist written permission to do so. There are exceptions to this: A court/judge may subpoena or order the release of information; if a client presents to be a danger to him/herself the therapist will assist to keep client safe; if a client shares their intention to kill someone, or shares their child/elder abuse experience, the therapist is required by law to report this information to the authorities.


Please be aware that we reserve the right to charge for any session not cancelled at least 24 hours in advance. This ensures we can continue to provide the best service possible to you and other clients.
